The Rural Health Transformation Program (RHTP) is a historic funding opportunity for rural Critical Access Hospitals (CAHs). From FY26 through FY31, CMS will distribute $50 billion across all 50 states, and based on our review of 35 state applications and budgets, roughly $10 billion is likely to be directed toward workforce.

The CAHs that say this out loud get taken seriously. Moreover, bring in regional partners: bring in your town’s LTC facility, home care agency, and community organizations: present a regional coalition, with you as the center, to serve local rural communities.

Strong proposals describe how the CAH will:

  • pull students from local high schools, Tribal communities, and regional colleges

  • host rotations, provide housing, train preceptors

Retention is not a perk. It is survival. RHTP reviewers know this, and they reward CAHs that know it too.

Reviewers can smell fluff a mile away. They want a plan that is sharp, real, and executable by a small, rural team.

Your proposal must show:

  • exactly who you need

  • why your region loses staff

  • how your pipeline will work

  • what you are doing to keep people

  • how you will measure success

  • how you will stop relying on travelers

No vague promises. No pretty diagrams. Just rural truth paired with rural solutions.
